Forensic Services Care Manager
Job Description:
Position Reports To:
Director of Forensic & Diversion Services
STATUS: Salaried-Exempt
PROGRAM: Forensic Services

DUTIES: Provides coordination of services regarding reintegration into the community of Forensic clients.

Provides Forensic Intervention Outreach Service to a maximum of 20 Clients at any one time, linking Forensic clients to entitlement and housing providers and providing outreach services to forensic clients requiring placement in other counties and states.

Maintains the integrity of the clinical record through timely and accurate completion of all required paperwork. Documentation required is program specific and may include, but is not limited to, treatment plans, assessments, psychosocials, SALs, progress notes, tracking forms, and intra-agency referrals. Employee shall submit paperwork to appropriate departments as required.

Serves as representative to Seminole County Sheriff’s Office, CIT and other community resource providers and develops resources and services for Forensic clients. Develops and maintains rapport with the Jail, Public Defender’s Office, State Attorney’s Office and Judges. Provides/participates in training to community and law enforcement agencies regarding Mental Health issues.

Completes services/visits to the jail as necessary.

Conducts assessments and completes referrals to appropriate services in a timely manner.

Assists all clients with initiating and/or reinstating benefits and assists Forensic clients with placement when not assigned to Case Management.

Assists clients with negotiating the criminal justice system and advocates for diversion when appropriate.

Serves as a representative to designated committees for community planning and development of activities and services.

Employee will complete trainings, attend and/or participate in meetings as required by SBH and/or regulatory agencies to ensure compliance with mandates, and to maintain certifications and licensure.


Employee maintains the integrity of the clinical record through timely and accurate completion of all required paperwork. Employees are responsible for ensuring they complete all required paperwork and training in order to receive a computer log-in to gain access to communication mediums used in conjunction with agency business.

Employees will possess at a minimum, basic computer literacy skills needed to utilize all communication mediums used in the conjunction with agency business to include but not limited to; agency intranet, on-line computer training, agency email, applicable software programs.

Employees will set up, maintain and use daily (where applicable) the following communication tools to keep abreast of agency updates; intranet, agency voice mail, and agency email.

In the provision of above services, the employee may need to transport a client based on priority service needs. Only Center-approved vehicles will be used by authorized employees to transport clients in accordance with Agency transportation policy.

Many clients are in severe emotional distress and employee must be able to communicate effectively and participate in both verbal and physical de-escalation techniques. Ability to excel in a stressful, constantly changing environment is required.

Adhere to all Agency Policies & Procedures.

Perform other duties as assigned by supervisor.

QUALIFICATIONS: Master’s Degree in Social Work, Criminology, Sociology, Psychology, or related Human Services field preferred, Bachelor’s Degree required. Two years experience with target population preferred.

ARNP
Job Description:
STATUS: Non-Exempt

Responsibilities may include Outpatient Medication Clinic, Crisis Stabilization Unit, Dual Diagnosis Programs.

1. Attends Team Meeting Monday – Friday and do initial assessments as needed.

2. Provides daily follow up with inpatients.

3. Performs all physicals, Monday – Friday.

4. Leads Group Therapy Sessions as determined by Unit Management Team (Medical Director, Nurse Manager & CSU Coordinator).

5. Leads Medication Discussion Groups on Crossroads as determined by Unit Management Team (Medical Director & Director of Substance Abuse Services).

OUTPATIENT MEDICAL CLINIC DUTIES 50% - 20 HOURS:

1. Provides follow up medication management appointments (15 minutes each) with stable clients.

2. Leads medication groups with stable clients, as determined by the Medical Director.

3. Attends Chronic/Case Management and General Therapy staffings, as determined by the Medical Director.

4. Completes psychiatric evaluations as determined by the Medical Director.

GENERAL DUTIES:

1. Attends weekly supervision meetings with the Medical Director.

2. Completes trainings as required by SBH, Regulatory Agencies, and as required for Licensure.

3. Ensures compliance with all regulatory agencies and standards.

4. Completes all required documentation in a timely and effective manner.

Registered Nurse - Full Time & Per Diem
Job Description:
SUPERVISOR: Nursing Manager


STATUS: Hourly

PROGRAM: Crisis Stabilization Unit

DUTIES: This is a professional nursing position on the Crisis Stabilization Unit, a secure adult psychiatric facility. Many clients are in severe emotional distress and employee must be able to communicate effectively and participate in both verbal and physical de-escalation techniques. Ability to excel in a stressful, constantly changing environment is required. This person is responsible for all issues related to patient care. Varying responsibilities may include the administration and ordering of medications and supplies, providing clinical assessments, staff supervision, program facilitation and other direct care concerns. Assigned duties would include either medication nurse or charge nurse responsibilities.

QUALIFICATIONS: Graduation from an accredited school of nursing and licensed by the State of Florida as a Registered Nurse. Able to maintain licensure without restrictions. Previous experience with target population preferred.
